Recommendation Form
The form below allows you to name a player or anyone associated with the keyboard world that, in your opinion, deserves an award for exceptional services to music and the public.
A guide on recommendations for awards by H.M Oueen is given on the page: QUEEN'S AWARDS (see Menu), briefly it says that recommendations should not be based on length of service but rather on what a person has done for others in addition to providing pleasure and entertainment - more than those providing the same service.
After naming the person on the form below, please give your reason for the recommendation in no more than fifty words.
Please note that a system exists to detect multiple submissions by an individual, and should this occur, all the relevant entries (including the first one) will be deleted. The system recognises the computer, the individual and location from which the recommendation is made. Only one submission is accepted from each personal computer/household.
No personal details are stored - they are only used for the purpose for which they are intended. You have the option of having an email sent to you of what you submit - just tick the appropriate box on the form.
