Insurance arrangements for the 2008/9 season

 It is important that all players are aware of the terms of the Insurance the club has put in place for the coming season. This is basic cover only.

In addition to the clubs insurance ALL players are strongly advised to take out their own personal accident Insurance.

 Procedures for making a claim under the Clubs Insurance: -

 The clubs insurers are the Leeds and District F.A. Accident Fund, who must receive any claim within 4 days of the occurrence of the injury.

 A completed claim form must be accompanied by a certificate signed by a medical practioner, clearly stating the nature of the injury and that the player is incapacitated from following or seeking employment.

 In addition the referee who officiated in the match where the injury was sustained must sign the claim form.

 FULL details of the claims procedure are available from the treasurer, but speed in obtaining the relevant documentation and a signature is vital in ensuring a successful claim.

 For successful claims a benefit of £36 per week will be paid for work absences up to a maximum of 24 weeks and subject to regular submission of fresh medical certificates.

 The Insuarnce relates to Cup,League and Friendly fixtures but does not cover training sessions.

  IMPORTANT, PLEASE NOTE IF YOU HAVE NOT PAID YOUR ANNUAL CLUB SUBCRIPTION YOU WILL NOT BE ENTITLED TO MAKE ANY CLAIM ON THE CLUBS INSURANCE
