1. Over-loaded beam.

It was a two-storey office building; the first floor was of beam-and-slab concrete.  One of the beams showed a vertical crack in the middle.

Upstairs was a wall-to-wall row of filing cabinets over that one beam.

It was suggested that they spread the load.
