This paper was originally prepared for delivery at the UNESCO/ICOMOS international congress, More Than Two Thousand Years in the History of Architecture: Safeguarding the Structures of Our Architectural Heritage, to be held in Bethlehem, (Palestine) January 22-25, 2001. For security reasons, the conference was unable to be held at that time. The conference was eventually held in Paris in 2001. The author was unable to attend and give the paper at that time.
